Update a current listing: If you wish to update a current listing, please click on the online version of the survey and log in using your name and email address.

By participating in the survey you will be contributing to our searchable Programs database.

  Consists of 7 sections. We estimate the time to complete the survey at 20 to 30 minutes.
• Print submission USA
  Download a paper version of the survey in a PDF file; complete it and mail or fax back to the TIE.
• Print submission NON-USA
  Download a paper version of the survey in a PDF file; complete it and mail or fax back to the TIE.

Please note that as part of an agreement with the National Library of Medicine (NLM), the TIE makes all of its telemedicine program data available to the NLM's Health Services Research Projects in Progress (HSRProj) database).
